We're looking for an exceptional and committed Regional (Branch) Extra Care Coordinator who'll be responsible for ensuring the operational and reputational success of our client's business across the Bromley area. Earning a competitive salary of up to £25,000, you'll report to the commercial director and work in close partnership with the regional lead nurse. Working full-time as a Extra Care Coordinator, you'll work 37.5 hours per week, to be worked flexibly, including evenings and weekends, according to the needs of the service and service users.
Our client is an independent healthcare organisation delivering exceptional quality of care to highly dependent adults and children in the comfort and security of their own homes throughout Bromley.
You'll be responsible for the achievement of service delivery and growth, in line with agreed performance and financial objectives.
Some of your Care Coordinator role and responsibilities:
? You'll be the contact and operational care lead, responsible for relationship building.
? You'll be responsible for multiple home-care services within the designated catchment area.
? You'll support the current client group of around 60 clients, and will play a lead role in growing the client list.
? You'll work closely with your regional (branch) lead nurse and the nurse case managers to achieve appropriate, safe, complex care and support for your clients.
Key Requirements to progress as a Care Coordinator include:
? Previous experience working with clients who qualify for continuing healthcare funding.
? Comprehensive knowledge of CQC assessment criteria for excellent and outstanding scores.
? Management experience within the healthcare sector alongside great collaboration skills.
