  • Current GMC registration (checked each time the Doctor is forwarded for work)
  • Verification of Right to work in the UK
  • Copies of all available vaccination and health records
  • Enhanced CRB dated within 12 months (and ISA)
  • Indemnity insurance where required
  • Competency in written and oral English
  • Appraisal information requested
